Appendix & Abdominal Emergencies

Gangrenous small bowel from loss of blood supply

Acute intestinal ischaemia can progress to gangrene and is a surgical emergency. Early recognition and prompt source control are critical because non-viable bowel may require urgent resection.

Surgical specimen after total thyroidectomy for multinodular goitre involving both thyroid lobes.

Clinical care

Total thyroidectomy specimen for multinodular goitre

A symptomatic multinodular goitre involving both lobes may require total thyroidectomy after appropriate evaluation. The extent of surgery depends on symptoms, anatomy, thyroid function and diagnostic findings.

Large neck swelling from goitre causing compression of the trachea.

Thyroid & Neck Surgery

Large goitre with tracheal compression

Many goitres are benign and can be observed after appropriate assessment. Surgery is considered when a large goitre causes compressive symptoms or other clinical concerns.

Diabetic Foot, Wounds & Burns

Diabetic foot after forefoot amputation and skin grafting

This patient required forefoot amputation for wet gangrene involving the toes. Continued wound care, pressure off-loading and later skin grafting achieved healing; long-term off-loading and diabetic foot surveillance remain important to reduce recurrent ulceration.

Chronic non-healing wound associated with lymphedema — before & after

Chronic wounds require treatment of the underlying cause, not dressings alone. In this selected case, prolonged compression therapy produced substantial improvement in a wound associated with longstanding lymphedema. Ongoing compression may be needed to reduce recurrence risk.

Healed supraclavicular wound after drainage of an abscess and secondary-intention healing.

Skin, Soft Tissue & Minor Surgery

Supraclavicular abscess — healing after drainage

Selected infected wounds are intentionally left open after drainage so they can heal by secondary intention rather than being closed immediately. The final scar can still be acceptable, depending on the wound and healing course.

Meckel diverticulum discovered during surgery

Meckel diverticulum is a congenital outpouching of the small intestine. It may be discovered incidentally or present with inflammation, bleeding or obstruction. Management depends on symptoms and operative findings.

Skin, Soft Tissue & Minor Surgery

Necrotising fasciitis after debridement and skin grafting

Necrotising fasciitis is a time-critical soft-tissue infection. Urgent removal of non-viable tissue is central to source control; reconstruction such as skin grafting is performed later once the wound is healthy enough.
